At Skin Dermatology in Shrewsbury, Massachusetts, we utilize electronic policies, privacy authorizations and consents. We do this to speed-up your check-in, streamline your visit and most importantly to make your life easier.

All electronic forms will be completed and signed electronically on our office provided iPads. These forms will then automatically save to your patient electronic health record. All of the forms utilized in our office are provided here for your review. The last thing we want is for you to feel rushed during your visit.
